Please bring the following documents with you to your local Neighborhood Partner Organization when you attend an Enrollment Workshop:

• Proof of Identity - Each adult must provide a photo identification card.
  Note: This does not have to be a government issued identification card.
  AND
• Proof of citizenship - Each adult must provide ONE of the documents from this list.
• Birth certificate showing U.S. citizenship
• U.S. passport
• Adoption record
• Permanent Resident Card, Form I-551 (Green Card)
• Naturalization certificate
• Letter from U.S. Citizenship and Immigration (CIS) (formerly INS) showing status and date of entry into the U.S.
• Military Service Record
• U.S. baptismal certificate listing where the adult was born
